Description
Medial exposure of the male leg frames the fibular (peroneal) vein as it ascends in the deep posterior compartment, closely applied to the posteromedial surface of the fibula and running with the fibular artery. Distally, tributaries from the lateral foot and ankle converge toward the posterior aspect of the lateral malleolus, then pass superiorly beneath the soleus, remaining lateral to the tibia and posterior to the interosseous membrane. Proximal continuity toward the tibioperoneal trunk is suggested, with the fibular vein joining the posterior tibial veins before contributing to the popliteal vein. Deep and paired veins dominate. This medial perspective matters because it clarifies how the peroneal veins sit behind the fibula and how easily they are missed when attention stays on the great saphenous system or the more medial posterior tibial veins. Calf deep vein thrombosis can involve the peroneal veins in isolation, and duplex ultrasound assessment often requires deliberate interrogation of this lateral deep channel, which is harder to compress and visualize than superficial veins. Surgeons also care about this relationship during fibular approaches and free fibular flap harvest, where the peroneal vascular pedicle and its venae comitantes must be identified and protected.
