Description
Running in the anterior compartment of the leg, the anterior tibial vein (often a paired venae comitantes) ascends from the dorsum of the foot toward the popliteal fossa, closely applied to the anterior tibial artery and deep fibular (peroneal) nerve. Its course lies anterior to the interosseous membrane and along the anterior intermuscular septum that partitions the tibialis anterior and extensor digitorum longus from the lateral compartment. Proximally it contributes to the popliteal vein, while distally it communicates with the dorsal venous arch and perforating veins from the superficial system. Valves segment the venous channel. Clinical relevance hinges on the deep versus superficial distinction in the male lower limb, because the anterior tibial vein participates in the deep venous system where acute deep vein thrombosis can propagate into the popliteal and femoral veins and raise pulmonary embolism risk. This relationship to the anterior tibial artery matters in tibial shaft fractures, anterior compartment fasciotomy, and anterior ankle approaches, where injury to venae comitantes can complicate hemostasis and postoperative swelling. Perforator connections also frame how superficial reflux, even when centered on the great saphenous vein, can load the deep system during prolonged standing. A small vessel with big consequences.
