Description
Running along the anterolateral compartment of the right leg, the anterior tibial vein is shown in lateral profile as a paired deep venae comitantes accompanying the anterior tibial artery. It courses proximally on the anterior surface of the interosseous membrane between tibia and fibula, deep to tibialis anterior and the long toe extensors, then passes superiorly toward the popliteal region. Distally, venous channels from the dorsum of the foot feed into the anterior tibial venous system, while more superficial venous pathways remain visible in the subcutaneous plane. A lateral view is a practical way to teach the deep course of the anterior tibial vein because it clarifies its relationship to the interosseous membrane and the bony partition that shields it from a purely anterior approach. This is the vein you need to remember when discussing anterior compartment decompression for acute compartment syndrome and when planning an anterolateral approach to the tibia, where iatrogenic injury to the anterior tibial neurovascular bundle can complicate fracture fixation. It also helps distinguish deep venous drainage from the great saphenous vein, which stays superficial and medial and follows a different set of clinical problems.
