Description
Running in the anterior compartment of the leg, the anterior tibial vein ascends from the dorsum of the foot toward the proximal leg as paired venae comitantes flanking the anterior tibial artery. Distally, it receives tributaries that accompany the dorsalis pedis vessels and communicate with superficial channels along the medial and lateral borders of the foot. As it courses superiorly, the vein lies anterior to the interosseous membrane and deep to tibialis anterior and the long toe extensors, then approaches the knee to join the posterior tibial veins and form the popliteal vein in the popliteal fossa. Deep, straight, and segmental. Clinically, this pathway sits at the center of deep venous thrombosis patterns that can extend from the calf into the popliteal system, and it helps explain why isolated calf DVT may present with anterior shin pain or minimal superficial findings. The anterior tibial vein also matters during anterior compartment surgery, tibial shaft fixation, and fasciotomy, where the neurovascular bundle (deep fibular nerve with anterior tibial vessels) is at risk if dissection drifts too deep or too medial. Venous anatomy here also underpins Doppler ultrasound mapping, since compressibility and flow assessment differ between paired deep veins and the superficial great saphenous system.
