Description
Running deep in the posterior compartment of the leg, the posterior tibial veins accompany the posterior tibial artery from the plantar foot proximally to the level of the distal popliteal fossa, where they contribute to the formation of the popliteal vein. Medially, their course parallels the tibia and passes posterior to the medial malleolus in the tarsal tunnel, where paired venae comitantes receive tributaries from the plantar venous plexus and the deep venous drainage of the calf. More superficially, the great saphenous vein ascends along the medial leg and thigh toward the femoral vein, making the superficial versus deep venous relationship clear in profile. Venous valves punctuate these channels. Deep venous anatomy in the lower limb matters because it is the substrate for most clinically significant thromboses, and the posterior tibial veins are routinely assessed on compression ultrasonography when evaluating suspected distal deep vein thrombosis. This view also clarifies why pathology in the tarsal tunnel can involve venous congestion along with tibial nerve symptoms, and why perforator incompetence can transmit high pressure from the deep system to superficial tributaries and varicosities along the medial leg. Small structures, big consequences.
