Description
Running along the deep posterior compartment of the male leg, the paired posterior tibial veins arise from the plantar venous arch in the sole and ascend posterior to the medial malleolus, closely accompanying the posterior tibial artery and tibial nerve. Proximally, they track superiorly along the posterior surface of the tibia, deep to the soleus and between the deep flexor muscles, with the fibula positioned lateral to the tibia as a clear bony reference. Superior to the ankle, the vessels head toward the popliteal fossa where the posterior tibial veins contribute to the formation of the popliteal vein, with the femur and patella providing proximal skeletal orientation in the same posterior view. For teaching venous return, this posterior perspective clarifies a point that is easy to confuse in text alone, the posterior tibial veins are deep venae comitantes, not the superficial great saphenous system.
