Description
Running deep in the posterior compartment of the leg, the posterior tibial vein courses inferiorly behind the tibia, paired as venae comitantes that closely flank the posterior tibial artery. Proximally, it lies deep to the soleus and gastrocnemius and becomes more accessible where the flexor digitorum longus and flexor hallucis longus frame the neurovascular bundle against tibialis posterior. Distally, the vessel approaches the medial malleolus, passing posterior to it in company with the artery and tibial nerve beneath the flexor retinaculum. Relationships are clear. Posterior tibial venous anatomy matters because it is a frequent site for deep vein thrombosis in the calf, and symptoms can be subtle until thrombus propagates proximally toward the popliteal vein. The illustration’s posterior perspective clarifies why duplex ultrasound interrogation must track the paired veins along the posterior tibial artery, and how the tight fascial boundaries of the deep posterior compartment influence venous compression, postoperative swelling, and the clinical picture in compartment syndrome.
