Description
Arising from the medial foot and leg and then sweeping proximally, the great saphenous vein is traced along the posteromedial calf and medial thigh as it ascends toward the saphenous opening at the groin. From this posterior angle, the vessel sits superficial to the deep fascia, crossing the posterior-medial contour of the knee region before continuing along the medial thigh, with small cutaneous tributaries and venules feeding into it. Surrounding landmarks include gluteus maximus superiorly and the posterior thigh compartments, with biceps femoris lying laterally and semitendinosus and semimembranosus more medially; adductor magnus forms a medial deep boundary in the proximal thigh. Color coding distinguishes venous channels (blue) from arteries (red) across the posterior buttock and thigh. Posterior mapping of the great saphenous vein matters because varicosities and reflux often involve the saphenous trunk and its thigh tributaries, and the pattern can be less intuitive when viewed from behind where perforators and superficial tributaries can appear to migrate around the medial border. This perspective also supports procedural planning for endovenous thermal ablation, foam sclerotherapy, and surgical high ligation and stripping near the saphenofemoral junction, where accurate surface anatomy reduces wrong-vessel treatment and incomplete closure.
