Description
Running subcutaneously along the posterior midline of the calf, the lesser saphenous vein arises from the lateral marginal vein of the foot and ascends behind the lateral malleolus toward the popliteal fossa. A posterior view also places the vein in relationship to the calcaneal (Achilles) tendon region inferiorly and the heads of gastrocnemius superiorly, with the fibula lateral and the tibia medial as deep osseous references. Near the knee crease, the vessel typically angles anteriorly to pierce the deep fascia and terminate in the popliteal vein. Perforating connections with the posterior tibial veins may be suggested. For venous anatomy teaching, this is the cleanest orientation to understand why symptoms and procedure planning differ from the great saphenous system on the medial leg. Endovenous ablation or foam sclerotherapy of the small (lesser) saphenous vein demands attention to its variable popliteal termination and to the close companionship of the sural nerve in the posterior calf, a known source of sensory complications when thermal energy tracks too superficially. Misidentifying the vein near the saphenopopliteal junction can also lead to incomplete treatment or recurrence through a Giacomini vein continuation into the posterior thigh. Anatomy matters here.
