Description
Running superficially up the posterior-lateral calf, the lesser saphenous vein (small saphenous vein, vena saphena parva) is traced from the lateral marginal vein of the foot, passing posterior to the lateral malleolus and ascending toward the popliteal fossa. From an anterior perspective, its course is read by proxy against the tibia and fibula and the contours of the gastrocnemius and soleus compartments, with the vein positioned lateral to the calcaneal (Achilles) tendon and posterior to the fibular shaft. Deep counterparts, including the anterior tibial, posterior tibial, and fibular (peroneal) veins, run with their companion arteries while the superficial channel remains closer to the skin. Appreciating the small saphenous vein’s shorter, calf-confined route matters when you plan treatment for venous reflux or map perforators for intervention. Endovenous ablation and phlebectomy commonly target this system, but the relationship to the sural nerve in the distal and mid-calf changes the risk profile compared with great saphenous work at the medial leg. Termination patterns also vary, with the classic saphenopopliteal junction in the popliteal fossa or proximal continuation as the vein of Giacomini, a frequent source of recurrence when missed. Educators can drop this plate into lower-limb anatomy blocks to contrast superficial and deep venous drainage, or into vascular surgery and interventional radiology materials when teaching duplex mapping, access planning, and complication avoidance around the popliteal fossa. It also fits well in patient-facing varicose vein counseling pieces where the difference between the lesser and great saphenous systems needs to be visually unambiguous.
