Great Saphenous Vein Viewed from Behind

Seen from a posterior perspective, both male lower limbs are rendered from the popliteal region to the calcaneus, with the great saphenous vein traced along the medial margin of the leg as it passes posterior to the medial malleolus before turning anteriorly toward the saphenous opening at the groin. Posteriorly, the small saphenous vein occupies the midline of the calf and courses superiorly toward the popliteal fossa, where it typically drains into the popliteal vein, while perforating veins bridge the superficial system to the deep venous channels accompanying the tibial and fibular arteries. Venous valves are indicated at intervals along the superficial trunk. Orientation is clear. This posterior mapping matters because reflux at the saphenofemoral junction or along the great saphenous trunk produces the familiar medial calf and thigh varicosities, and treatment planning depends on distinguishing great saphenous disease from small saphenous or perforator incompetence. The medial malleolar segment is a key surface landmark for duplex ultrasound and for correlating symptoms of chronic venous insufficiency, including edema and stasis dermatitis, with the underlying venous pathway rather than the deep system implicated in acute deep vein thrombosis.
