Posterior to the cranial base, the condylar emissary vein is traced as it traverses the condylar canal adjacent to the occipital condyle, linking the sigmoid sinus region internally with the vertebral venous plexus and suboccipital venous network externally. Surrounding landmarks include the occipital bone and the posterior arch of the atlas, with the vessel running deep to the suboccipital musculature and near the margins of the foramen magnum. Superficially, the trapezius and splenius capitis frame the field, while the sternocleidomastoid defines the posterolateral neck. Small caliber, big consequences. Emissary veins matter because they create valveless conduits between intracranial dural venous sinuses and extracranial veins, and the condylar emissary vein is one of the most variable but surgically relevant of these channels. Posterior approaches to the craniovertebral junction (foramen magnum decompression, occipital condyle drilling, C1 lateral mass instrumentation) can encounter brisk venous bleeding when this vein or its plexiform connections are opened at the condylar canal. Infection can also track retrograde along these pathways, a mechanism invoked in posterior scalp or upper neck sources contributing to dural sinus thrombophlebitis. This view makes the venous route concrete.
