Description
Centered on the posterior cervical region, the vertebral vein descends bilaterally from the suboccipital area and tracks inferiorly along the cervical transverse processes, corresponding to its passage through the transverse foramina from C1 to C6. Surrounding it, the deep posterior neck musculature is layered, with splenius capitis and semispinalis capitis superficial to the short suboccipital muscles near the occiput, and the upper trapezius forming a more superficial sheet inferiorly. Segmental dorsal rami of the cervical spinal nerves course posterolaterally between these muscle planes near the articular pillars. Clear midline landmarks. Posterior relationships matter because the vertebral vein communicates with the suboccipital venous plexus and vertebral venous plexuses, providing a low pressure, valveless route that can transmit infection or metastatic spread between the cranial base and cervical spine. This view also helps when teaching why posterior cervical surgical exposure, needle placement, or aggressive retraction near the transverse processes can produce troublesome venous bleeding, even when the vertebral artery is the structure everyone worries about. The close packing of veins, dorsal rami, and deep muscles around C1 to C2 is where anatomy stops being forgiving. Ideal for gross anatomy and head and neck modules that cover vertebral vessels in parallel with the cervical plexus and posterior triangle boundaries, and for surgical education materials discussing posterior approaches to the upper cervical spine or suboccipital decompression where venous plexus management shapes the operative field.
