Description
Arising in the deep posterior compartment of the leg, the posterior tibial vein tracks proximally from the plantar venous plexus behind the medial malleolus, then ascends on the posteromedial surface of the tibia in tight company with the posterior tibial artery. From a medial perspective, the vein sits posterior to the tibial shaft and anterior to the flexor compartment musculature, with the fibula remaining lateral and partially obscured by the tibia. Proximally, its course leads toward the popliteal fossa where the paired venae comitantes converge with the fibular (peroneal) veins to form the popliteal vein. Deep venous anatomy is the focus. Clinical relevance is practical and specific: the posterior tibial veins are a standard target in duplex ultrasound when evaluating suspected deep vein thrombosis in the calf, and their close association with the posterior tibial artery helps with identification and compressibility testing. The retromalleolar segment, coursing between the medial malleolus and calcaneal tendon, also matters during ankle surgery and medial approaches to the hindfoot, where iatrogenic injury can complicate hemostasis and postoperative swelling. For vascular access planning, this view clarifies why superficial veins such as the great saphenous do not substitute for the deep drainage pathway. Suitable applications include lower-limb anatomy teaching in gross anatomy and sonography curricula, as well as surgical atlases covering tibial fixation, tarsal tunnel exposure, and posterior compartment fasciotomy landmarks; it also supports patient education materials explaining calf DVT and venous return from the foot.
