Description
Running in the deep posterior compartment of the leg, the posterior tibial vein ascends from the plantar venous plexus and courses superiorly just posterior to the tibia, typically as paired venae comitantes that flank the posterior tibial artery. Medially, the great saphenous vein (long saphenous) tracks anterior to the medial malleolus and continues up the medial border of the leg, separated from the deep system by the deep fascia except where perforating veins connect them. Proximally, the posterior tibial vein passes beneath the tendinous arch of soleus (soleal sling) before contributing to the tibioperoneal trunk and then the popliteal vein. Venous valves appear along the deep and superficial channels. A tight anatomical corridor. Clinically, the segment beneath the soleus arch matters because calf muscle compression and the soleal venous sinuses create a common site for thrombus formation, with clot extension tracking into the posterior tibial veins and onward to the popliteal vein in proximal deep vein thrombosis. This relationship also guides duplex ultrasound technique: compressibility and flow augmentation are assessed along the posterior tibial veins, and limited calf DVT studies can miss isolated posterior tibial thrombi if the operator does not interrogate below the soleal sling. The adjacent tibial nerve and posterior tibial artery define the neurovascular bundle surgeons protect during posterior approaches to the ankle and distal leg.
