Description
Running deep along the posterior aspect of the leg, the fibular (peroneal) artery is traced from its origin off the posterior tibial artery in the proximal leg as it descends in the deep posterior compartment adjacent to the medial surface of the fibula. Nearby, the tibia lies medial and the fibula lateral, framing the vessel as it courses inferiorly toward the ankle, where perforating and malleolar branches contribute to the lateral calcaneal and tarsal arterial networks. Superficial and deep calf musculature are logically represented in posterior relation, with gastrocnemius and soleus overlying the deeper plane that includes tibialis posterior and flexor hallucis longus. It sits where you expect it. Posterior exposure matters because the fibular artery is often spared in atherosclerotic disease compared with the anterior and posterior tibial arteries, making it a common target for distal bypass or endovascular revascularization in chronic limb-threatening ischemia. The relationship to the fibula and deep flexor muscles also explains why iatrogenic injury can occur during posterolateral approaches for fibular fracture fixation, and why septocutaneous perforators from the fibular artery are dependable for fibula osteocutaneous free flaps in head and neck reconstruction. Seeing the vessel in situ helps clarify how collateral flow reaches the foot when one of the other crural arteries is occluded.
