Description
Running in the deep posterior compartment of the male leg, the flexor digitorum longus (FDL) is highlighted along the posterior surface of the tibia, positioned medial to the flexor hallucis longus and deep to the soleus and gastrocnemius. Its muscle belly arises proximally from the posterior tibia, then narrows distally into a tendon that courses inferiorly toward the medial malleolus. Medial and slightly anterior to the Achilles tendon, the FDL tendon passes behind the tibia and into the plantar foot, where it will divide into slips for the lateral four toes. The tibia forms the main bony landmark, with the fibula lying lateral to the deep flexor group. Clinically, this posterior view matters because the FDL shares the retromalleolar space with the tibialis posterior tendon and the posterior tibial neurovascular bundle, all constrained by the flexor retinaculum. Tightness, tenosynovitis, or space-occupying lesions here can contribute to tarsal tunnel syndrome, with pain and paresthesia radiating into the plantar foot. Surgeons also encounter the FDL in tendon transfer procedures for posterior tibial tendon dysfunction, where its line of pull and proximity to the navicular insertion of tibialis posterior make it a practical donor.
