Description
Posterior thoracic anatomy is presented with the thoracic vertebral column centered and the ribs (costae) sweeping laterally from the costovertebral and costotransverse joints. Along the posterior shafts, each rib curves through its costal angle before continuing anterolaterally, while the tubercle region sits immediately lateral to the transverse processes as a palpable landmark for rib counting. A left scapula overlies the upper posterior cage, with its medial border and inferior angle projecting superficial to ribs 2 through 7 in typical anatomical position. Fracture markers concentrate near the posterior angles and tubercles, a common location for bending stress injuries. Posterior rib fractures matter because they sit close to the neurovascular bundle in the costal groove on the inferior rib margin, where jagged fragments can lacerate the intercostal artery or nerve and drive pain-limited ventilation. This is the pattern often seen after blunt trauma from a fall or direct blow to the back, and it correlates with pulmonary contusion or occult pneumothorax that can be missed on a supine anteroposterior radiograph. Posterior breaks near the costotransverse region also help explain focal tenderness just lateral to the spinous processes, where examination and ultrasound can be awkward under the scapular blade. Pain dominates. Use this posterior perspective for teaching rib morphology (head, neck, tubercle, angle) in gross anatomy and for correlating trauma imaging, including oblique rib series and CT reconstructions that localize fracture lines for surgical stabilization of rib fractures (SSRF). It also reads well in emergency medicine, orthopaedics, and thoracic surgery content discussing intercostal nerve blocks, chest tube placement planning, and the mechanics of flail chest.
