Posteriorly, the paired fourth ribs (costae IV) arc laterally from the thoracic vertebral column, with their heads seated against the costal facets at the T4 vertebral body and the tubercles approximating the transverse processes at the costotransverse joints. Medial rib segments sit immediately lateral to the spinous processes and laminae, then sweep posterolaterally as the shafts broaden and curve toward the lateral thoracic wall. Intercostal spaces above and below the fourth rib are clearly defined, and the inferior thoracic vertebrae transition to the lumbar vertebrae before the sacrum and posterior iliac crests enter the inferior margin of the frame. Select ribs are color-highlighted to isolate the fourth level. Clean bony landmarks. A posterior emphasis on the fourth rib is a practical way to teach rib-vertebra articulation: the head’s relationship to the vertebral body and the tubercle’s relationship to the transverse process are easy to confuse when learners only see an anterior ribcage. Clinically, this level sits near the inferior angle of the scapula in many adults, so it helps with surface marking for posterior thoracic procedures and for describing fracture location (posterior angle versus costovertebral region), where pain can mimic paraspinal pathology. Costovertebral and costotransverse joint irritation can also refer pain along the corresponding intercostal nerve. Orientation matters.
