Description

Seen from the anterior thorax, the right and left third ribs (costae III) arc laterally from their costochondral junctions toward the axillary line, forming the superior portion of the true rib cage above ribs IV to VII. Each rib’s head and neck lie posteriorly at the thoracic vertebral column, with the tubercle approaching the transverse process, while the shaft curves anterolaterally and then turns forward toward the sternum via its third costal cartilage. Superiorly, the first and second ribs sit closer to the thoracic inlet; inferiorly, the fourth rib begins the next intercostal space. Intercostal spaces 2 and 3 frame the rib, bounded by the inferior margin of rib II and the superior margin of rib IV. Orientation at the third rib matters in both surface anatomy and trauma. The third intercostal space is a common reference when teaching parasternal auscultation landmarks and interpreting chest radiographs, since rib counting often begins at the palpable sternal angle (Angle of Louis) at the level of the second costal cartilage, then proceeds inferiorly. Rib fractures in the upper true ribs raise concern for higher-energy mechanisms, and the posterior third rib region sits near the costovertebral and costotransverse joints where pain can localize in thoracic spine referral patterns.
