Positioned in lateral profile, the right and left second ribs (costae II) arc from the thoracic vertebral column anteriorly toward the manubrium, with their heads and tubercles articulating posteriorly at the level of the second thoracic vertebra and the costotransverse joint. Each shaft curves anterolaterally, then turns medially as it approaches its costal cartilage, which runs toward the sternum and contacts it indirectly through the adjacent cartilage at the sternal angle region. Posterior elements of the spine, including thoracic vertebral bodies, intervertebral discs, and the gentle thoracic kyphosis, sit medial and posterior to the rib cage. Blue highlighting draws attention to the upper ribs, allowing the second rib to be picked out against the surrounding costae. Second rib anatomy matters because it is a dependable surface landmark for counting ribs and intercostal spaces, anchored clinically by the sternal angle (Angle of Louis) at the manubriosternal joint.
