Description
Lateral osteology of the adult male thorax isolates the second rib (costa II), highlighted against adjacent ribs, as it arcs from its posterior articulation at the second thoracic vertebra to its anterior costal cartilage. Posteriorly, the rib head meets the costal facets on T1 and T2, and the tubercle aligns with the transverse process of T2 at the costotransverse joint, while the shaft curves anteroinferiorly and then turns anteriorly toward the sternum. Superior to it, the first rib appears shorter and more horizontal, so the second rib’s longer sweep helps define the upper contour of the rib cage relative to the thoracic kyphosis seen in the accompanying vertebral column. Intervertebral discs are visible between thoracic vertebral bodies and continue inferiorly toward the sacrum. Clinically, the second rib and the second intercostal space are frequent surface anatomy reference points because the sternal angle (Angle of Louis) marks the level of the second costal cartilage, guiding rib counting on physical exam, chest radiographs, and CT. Its relationship to T2 also matters in trauma patterns: posterior fractures near the costovertebral or costotransverse joints can be subtle on plain films but can contribute to pain with respiration and reduced chest wall mechanics. A clean lateral view helps teach why costa II has a more typical curvature than the first rib, which often confuses learners when estimating rib levels.
