Description
Visible in the upper thorax are multiple costae (ribs) curving anterolaterally from the thoracic vertebrae toward the sternum, with costal cartilages bridging the anterior gaps to the manubrium and body of the sternum. Several ribs demonstrate acute fracture lines with jagged cortical margins along the costal arch, marked at the mid to anterolateral segments where bending stresses commonly concentrate. Superiorly, the clavicle spans from the sternoclavicular region toward the acromion, and the scapula sits posterolateral to the rib cage, framing the glenohumeral joint with the proximal humerus. Portions of the thoracic vertebral column and intervertebral discs remain visible posteriorly, providing clear rib-to-vertebra relationships. Rib fractures are not just a bony injury; they are a proxy for chest wall instability and pleural risk, and this view makes it easy to correlate fracture location with adjacent structures such as the intercostal neurovascular bundle along the inferior rib margin. Posterolateral fractures near the angle of the rib often follow blunt trauma and can be missed on supine radiographs, while more anterior breaks raise practical concerns for pain-limited ventilation and atelectasis. A critical teaching point is mechanism: a single displaced fracture behaves differently from serial fractures that can produce a flail segment and paradoxical motion, even when the sternum and costal cartilages appear intact. Pain matters.
