Description
Running longitudinally along the posterolateral neck, the iliocostalis cervicis appears as the lateral column of the erector spinae, positioned lateral to spinalis cervicis and longissimus cervicis and deep to the superficial fascia of the male upper back. Its slips ascend from the angles of the upper ribs to insert on the posterior tubercles of the transverse processes of C4 to C6, lying posterior to the cervical transverse processes and lateral to the spinous process line. Superiorly, the muscle approaches the occipital region but does not attach to the cranium, while inferiorly it blends into the thoracic iliocostalis. Adjacent landmarks include the scapulae laterally and the upper thoracic ribs inferiorly. Clean, readable anatomy. Posterior cervical myofascial pain commonly localizes to the lateral erector spinae mass, and iliocostalis cervicis trigger points can refer discomfort toward the lower neck and shoulder girdle, a pattern often reported after whiplash-type acceleration injuries. The insertions onto the cervical transverse processes make this view useful when teaching segmental control and coupling mechanics during cervical extension and ipsilateral lateral flexion, and when differentiating deep paraspinal sources of pain from levator scapulae strain. The highlighted levator scapulae, running from the transverse processes to the superior angle and medial border of the scapula, helps clarify why scapular elevation and downward rotation can amplify perceived neck pain even when the primary tenderness sits in the erector spinae.
