Oriented from a posterior perspective, the cecum sits inferior to the terminal ileum and proximal ascending colon, with the vermiform appendix projecting from the inferomedial cecal wall. Along the posterior surface of the cecum, the posterior cecal artery courses close to the tenia coli and wraps posterolaterally before sending small branches into the cecal wall around the haustra. Blue venous tributaries accompany the arterial arcade, draining toward the ileocolic venous system. Attention to the posterior cecal artery matters because this vessel is encountered where the cecum and appendix are mobilized off the retroperitoneum, and its branches can be a source of troublesome bleeding when the peritoneal reflection is incised. Appendectomy and right hemicolectomy both place the ileocolic pedicle and its cecal branches at risk, and the posterior cecal branch may be less obvious from an anterior approach because it runs on the back of the cecum. Clear depiction of the haustra, taeniae, and appendiceal base helps link vascular supply to common landmarks used to find and ligate the mesoappendix.
