Arising most commonly from the ileocolic artery, the appendicular artery runs within the mesoappendix to reach the vermiform appendix, where it courses distally toward the tip with only limited collateral input. From a posterior perspective, the cecum sits superior and medial to the appendix base, while the mesoappendix fans posteriorly from the terminal ileum and cecal region to cradle the appendicular vessels. Blue appendicular veins accompany the artery along the same mesenteric fold, and small ileocolic and appendiceal lymph nodes cluster along the vascular pedicle near the appendix root. That posterior view matters in appendectomy because the surgeon typically mobilizes and divides the mesoappendix to control the appendicular artery, and its variable origin or early branching can hide behind the cecum or terminal ileum.
