Description

Running along the midline of the penile dorsum, the deep dorsal vein of the penis is shown in anterior orientation, flanked by the paired dorsal arteries and bordered laterally by the dorsal nerves. Superficial to Buck fascia, the superficial dorsal vein courses in the subcutaneous tissue toward the pubic region, while deep erectile tissues, the corpora cavernosa and corpus spongiosum, are opened to clarify internal drainage pathways. Proximally, venous channels converge toward the suspensory region of the penis and pubic symphysis; distally, they approach the glans penis and the distal shaft. Venous outflow and dorsal neurovascular anatomy matter when you are teaching the hemodynamics of erection and the mechanisms of detumescence, but they also come up in day-to-day urologic practice. Injury to the deep dorsal vein complex and adjacent dorsal arteries is a predictable bleeding source during penile degloving, circumcision revision, and dorsal plication for Peyronie disease, and it guides dissection planes during penile prosthesis surgery. Sensory pathways shown along the dorsum correlate with dorsal nerve block technique and help explain altered penile sensation after trauma or iatrogenic injury. Small structures. Tight real estate.
