Description

Positioned on the posterior aspect of the right lower quadrant, the posterior cecal artery is shown coursing along the back wall of the cecum near the ileocecal junction. Branches track inferiorly toward the base of the cecum and superiorly toward the cecal pole, running in close association with small cecal veins that mirror the arterial path. Portions of terminal ileum and the proximal ascending colon with haustral contour sit adjacent, while mesenteric connective tissue carries accompanying autonomic nerve fibers and regional lymphatics. Small-caliber arterioles ramify into the posterior cecal wall. Clear orientation. Understanding the posterior cecal arterial supply matters when correlating right iliac fossa pain with operative findings, because a retrocecal or pelvic appendix often shares this vascular neighborhood via branches of the ileocolic artery. During appendectomy or right hemicolectomy, traction on the cecum and dissection near the ileocecal fold can avulse small posterior cecal branches and produce oozing that is easy to miss from an anterior exposure. This posterior perspective also supports teaching of watershed risk and localized ischemia in the cecal wall when ileocolic inflow is compromised, a scenario encountered in low-flow states and after ligation choices.
