Description

Running along the posteromedial thigh, the semitendinosus forms one of the superficial hamstring contours from the ischial tuberosity inferiorly toward the medial aspect of the knee. From a posterior perspective you would expect to see it lying medial to the biceps femoris long head and superficial to the semimembranosus, with its long distal tendon tracking toward the pes anserinus on the proximal medial tibia. The pelvic ring is present superiorly, including the ilium, ischium, pubis, sacrum, and coccyx, with the femur descending to the knee where the posterior femoral condyles relate to the popliteal fossa. Clear lines of pull matter here. Clinically, this posterior hamstring view supports teaching of tendon harvest for anterior cruciate ligament reconstruction, where the semitendinosus (often paired with gracilis) is commonly stripped through an anteromedial proximal tibial incision and must be distinguished from the more anterior sartorius fascia and from the nearby saphenous nerve. Strain injuries also cluster at the proximal myotendinous junction near the ischial tuberosity, and an accurate depiction helps explain why symptoms can mimic proximal hamstring tendinopathy or ischial bursitis. The relationship to the knee is just as relevant, since semitendinosus contributes to knee flexion and internal rotation and can influence dynamic valgus control in rehabilitation planning.
