Description
Oriented from a posterior perspective, the distal femur sits superior to the proximal tibia, with the intercondylar fossa opening toward you and the tibial plateau forming the inferior articular surface. Crossing within the joint, the anterior cruciate ligament and posterior cruciate ligament form the familiar central decussation, while the posterior capsule thickens into the oblique popliteal ligament and arcuate popliteal ligament spanning superolaterally from the fibular head region toward the capsule. Along the medial and lateral margins, the tibial (medial) collateral ligament and fibular (lateral) collateral ligament outline the joint line, and the posterior horn regions of the medial and lateral menisci lie deep at the tibial plateau. The patella is anterior and largely out of plane. Posterior knee anatomy is where stability becomes three-dimensional. This angle makes the PCL’s course from the lateral surface of the medial femoral condyle to the posterior intercondylar area of the tibia easy to teach, and it frames how posterolateral corner structures resist varus stress and external tibial rotation when the cruciates are intact or deficient. It also maps cleanly onto clinical injury patterns, such as dashboard trauma causing PCL rupture, or combined PCL and posterolateral corner injury leading to persistent posterior sag and rotational instability if the corner is missed at reconstruction.
