Description
Centered on the gastroduodenal junction, the distal stomach narrows into the pyloric antrum and pyloric canal, where the pyloric sphincter is shown relaxed and fully patent as it opens into the first part of the duodenum. Gastric rugae line the lumen proximally and flatten toward the pylorus, while the duodenal mucosa beyond the valve shows the onset of plicae circulares. Encircling the lumen, the thickened circular smooth muscle of the pyloric sphincter sits at the distal end of the stomach, continuous with the muscularis externa of the duodenum. A relaxed open pylorus is a teaching-critical counterpoint to the contracted valve you see in most diagrams, because it clarifies how gastric chyme exits the stomach in pulses while antral peristaltic waves drive trituration and controlled emptying. This anatomic moment is central to understanding gastric outlet obstruction and hypertrophic pyloric stenosis, where failure of pyloric relaxation and muscular thickening narrow the channel and produce nonbilious projectile vomiting. It also frames why endoscopists target the pyloric ring as a landmark when entering the duodenal bulb and evaluating ulcer disease at the pylorus and proximal duodenum.
