Description
Arising from the cranial base, the paired optic nerves course anteriorly through the optic canals toward the posterior poles of the ocular globes, where the retinal circulation originates from the ophthalmic artery system. Medial to each orbit, anterior and posterior ethmoidal vessels track along the ethmoidal foramina at the frontoethmoidal suture line, linking the orbit to the nasal cavity and anterior cranial fossa. Superior to the globes, the superior ophthalmic vein runs posteriorly toward the cavernous sinus, receiving tributaries that include the ethmoidal veins and the angular vein via the medial canthus. Bony landmarks of the skull base remain in view, including the sphenoid body around the optic chiasm region and the centrally placed foramen magnum inferiorly. Vascular and neural relationships in this region matter because the orbit has direct venous communication with intracranial dural sinuses and lacks valves in key channels. Infection from the face or paranasal sinuses can propagate through the superior ophthalmic vein to the cavernous sinus, producing cavernous sinus thrombosis with ophthalmoplegia from involvement of cranial nerves III, IV, V1, V2, and VI. Ethmoidal arterial branches also explain why endoscopic sinus surgery near the cribriform plate and anterior ethmoidal artery can result in brisk orbital hemorrhage or, in rare cases, intracranial complications. Small vessels. Big consequences.
