Description
Running along the posteromedial thigh, the semimembranosus appears as a broad, flat muscle belly deep to the more superficial semitendinosus, with its proximal tendon arising from the ischial tuberosity and its distal fibers narrowing as they pass toward the medial tibia. From a medial perspective under intact skin, the muscle sits posterior to the adductor magnus and medial to the biceps femoris, while the gracilis and sartorius form a more anterior, superficial contour along the medial thigh. Distally, the semimembranosus tendon courses posterior to the medial femoral condyle toward its insertion on the posteromedial tibial condyle, adjacent to the region of the pes anserinus tendons. This view matters because the semimembranosus is the hamstring most often implicated in deep posteromedial knee pain and in the formation of a semimembranosus bursa distention that can communicate with the knee joint, the common anatomical substrate of a Baker cyst.
