Description
Medial anatomy of the posterior thigh is organized around the semitendinosus, highlighted here as a long, fusiform hamstring running inferiorly from the ischial tuberosity toward the medial aspect of the knee. It lies superficial and slightly posterior to the broader semimembranosus, while the biceps femoris sits more lateral in the hamstring compartment along the linea aspera side of the femur. Distally, the semitendinosus narrows into a cordlike tendon that courses anteromedially to reach the proximal tibia, just inferior to the medial tibial condyle, in the region of the pes anserinus. Fiber direction and fascial planes make the superficial depth relationship easy to read. For teaching and operative planning, this medial view clarifies why semitendinosus is often the more accessible hamstring: its tendon becomes distinct early, separates cleanly from semimembranosus, and can be followed to its insertion adjacent to the gracilis and sartorius tendons. That anatomy matters during autograft harvest for anterior cruciate ligament reconstruction, where accessory slips, premature bifurcation, or adhesions near the pes anserinus can complicate tendon stripping and increase the risk of iatrogenic injury. Palpation landmarks also track well to pathology.
