Seen from a posterior perspective, the occipital belly of the occipitofrontalis (epicranius) spans the occipital scalp, its muscular fibers arising from the lateral two thirds of the superior nuchal line and the mastoid region before fanning superiorly to blend into the epicranial aponeurosis (galea aponeurotica). Laterally, the occipitalis approaches the auricle and sits superficial to the occipital bone and the upper fibers of trapezius, separated by the dense connective tissue layers of the scalp. Inferior to the hairline, the external occipital protuberance and nuchal ligament region provide reliable midline landmarks, while the posterior cervical musculature frames the lower edge of the field. Functionally, this posterior view matters because the occipital belly tensions the galea to coordinate scalp movement with the frontal belly, and its tethering to the nuchal line explains the characteristic gaping of scalp lacerations when the aponeurosis is violated. Facial nerve anatomy also enters the conversation: the occipital belly is supplied by the posterior auricular branch of cranial nerve VII, so postoperative weakness after retroauricular dissection or parotid surgery can present as subtle loss of posterior scalp movement rather than obvious brow ptosis. A small muscle, a big clue. The same tissue planes define the potential subgaleal space, where hemorrhage can spread widely over the calvaria in neonatal vacuum extraction injuries and blunt trauma.
