Description
Anteriorly, the colon is rendered with multiple epiploic appendages (appendices epiploicae) projecting from the serosal surface, most conspicuous along the transverse and sigmoid segments where they hang inferiorly and slightly anterior to the tenia coli. Loops of jejunum and ileum occupy the central abdomen, suspended by mesentery whose root courses obliquely from the left upper quadrant toward the right iliac fossa. Red arterial branches and blue venous tributaries track within the mesenteric folds toward the bowel wall, while the greater omentum lies superficially and can be appreciated as a fatty drape anterior to the intestinal mass. Small. Fatty. Pedunculated. This angle matters because epiploic appendages have a narrow vascular stalk and can undergo torsion with venous thrombosis, producing epiploic appendagitis, a focal cause of acute left or right lower quadrant pain that often mimics diverticulitis or appendicitis on presentation. Correlating their distribution along the colon helps explain why pain localizes near the sigmoid or cecum and why CT often shows a small, fat-density ovoid lesion with a hyperattenuating rim adjacent to the colon. The relationship to the omentum and mesentery also clarifies why surgeons distinguish an inflamed appendage from omental infarction during diagnostic laparoscopy. Ideal for teaching peritoneal anatomy in gross anatomy and GI blocks, and for illustrating radiology or emergency medicine content on epiploic appendagitis, diverticular disease, and differential diagnosis of acute abdominal pain in adult men.
