Description
Foregrounded on the anterior surface of the colon, multiple epiploic appendages (appendices epiploicae) project as small adipose pouches along the taeniae coli, most apparent on the transverse and sigmoid segments. Superiorly, the liver occupies the right upper quadrant with the gallbladder tucked on its inferior surface, while the stomach and spleen sit to the left, partially overlapping the upper margin of the transverse colon. Central abdominal space is filled by coils of small intestine, framed laterally by the ascending colon on the patient’s right and the descending colon on the left. Mesenteric vessels and regional lymphatics accompany the bowel in the background, with the greater omentum positioned anterior to portions of the intestinal mass. Attention to the epiploic appendages matters because they are the substrate for epiploic appendagitis, a torsion or venous thrombosis of an appendage that produces focal, nonmigratory lower quadrant pain and is often misdiagnosed as acute appendicitis or diverticulitis. On CT, the inflamed appendage appears as an ovoid fat-density lesion adjacent to the colon with a hyperattenuating rim and sometimes a central dot sign representing a thrombosed vein, a pattern that changes management toward conservative treatment. Their segmental distribution also helps explain why symptoms cluster around the sigmoid and cecal regions. Ideal for gross anatomy and GI radiology teaching where learners need an anterior reference of colon surface anatomy and peritoneal fat structures, including the relationship between taeniae coli, epiploic appendages, and the greater omentum. It also supports surgical and emergency medicine texts discussing acute abdominal pain differentials and avoids the common pitfall of treating epiploic appendagitis as diverticulitis.
