Description
Running along the greater curvature of the stomach, the left gastroepiploic (left gastro-omental) vein is presented from an anterior perspective in an adult male, tracking within the layers of the greater omentum. Proximally it relates to the gastric fundus and inferior aspect of the cardia, then continues inferiorly and to the right along the anteroinferior border of the gastric body toward the antrum and pylorus, with the duodenum beginning distally. The esophagus enters superiorly at the gastroesophageal junction, providing a clear topographic anchor for the venous course on the anterior gastric surface. Small venous tributaries (venules) converge toward the main channel as it parallels the greater curvature. Emphasis on the left gastroepiploic vein matters because it sits in the operative field during mobilization of the greater curvature and division of the gastrocolic ligament, where bleeding can be brisk if the vessel is avulsed rather than controlled. Surgeons also reference this vessel when preserving the right gastroepiploic artery and vein as a pedicle for gastric conduit formation after esophagectomy, and when planning lymphadenectomy along the greater curvature for distal gastric cancer. It clarifies where the greater omentum and gastroepiploic arcade are encountered and why careful ligation is preferred over blind energy application near the pylorus and antrum.
