Description
Framed from an anterior perspective, the distal tibia sits medial to the distal fibula, forming the ankle mortise around the trochlea of the talus, with the dorsum of the tarsus and metatarsals continuing distally to the phalanges. Across the anterior ankle, the superior and inferior extensor retinacula span from lateral to medial, tethering the extensor tendons against the underlying talus and calcaneus. Distal to the retinacula, dorsal tarsal ligaments and the dorsal tarsometatarsal bands bridge the midfoot joints, running between the cuneiforms, cuboid, and bases of the metatarsals. The layout makes the medial column (talus, navicular, cuneiforms, first metatarsal) read distinctly from the lateral column (calcaneus, cuboid, fourth and fifth metatarsals). An anterior rendering like this earns its keep when teaching dorsal midfoot stability. The dorsal ligaments are thinner than their plantar counterparts, yet they define the joint lines surgeons target for dorsal approaches to the talonavicular and tarsometatarsal (Lisfranc) articulations, and they help explain why subtle diastasis can appear on stress radiographs after a Lisfranc sprain. Retinacular anatomy matters too, since extensor retinaculum thickening or post-traumatic scarring can contribute to anterior ankle pain and tethering of the extensor hallucis longus and extensor digitorum longus during rehabilitation. Clear landmarks.
