Description
Dorsally, the tarsometatarsal (Lisfranc) joint line is defined by the distal tarsals, the medial, intermediate, and lateral cuneiforms and the cuboid, articulating with the bases of metatarsals I to V. A network of dorsal tarsometatarsal ligaments spans these joints as short, obliquely oriented bands that blend with the dorsal joint capsules and course from the tarsal bones distally to the metatarsal bases, with stout slips typically concentrated over the medial and central columns. Extensor tendons traverse superficial to the complex, including extensor hallucis longus medially and extensor digitorum longus more laterally, crossing the region en route to the toes. The ankle mortise and adjacent hindfoot are included for orientation, with the tibia and fibula superior to the talus. This dorsal perspective matters because subtle malalignment at the tarsometatarsal joints can destabilize the midfoot even when the plantar Lisfranc ligament is the principal restraint. Dorsal ligament disruption accompanies common injury mechanisms such as axial load on a plantarflexed foot, and dorsal swelling and ecchymosis often track across this surface in acute Lisfranc trauma.
