Description
Seen in lateral profile, the distal fibula (lateral malleolus) overlies the talus and forms the lateral wall of the talocrural joint, with the calcaneus positioned posterior and inferior to the talar body. The anterior talofibular ligament (ATFL) runs anteromedially from the anterior margin of the lateral malleolus to the lateral aspect of the talar neck, lying superficial to the ankle joint capsule and anterior to the subtalar joint line. Adjacent tarsal elements of the lateral column, including the cuboid and the bases of the metatarsals, provide distal context for hindfoot to midfoot alignment. A small structure with outsized clinical consequences. Because the ATFL is the weakest component of the lateral collateral ligament complex, it is the first restraint to fail during inversion with plantarflexion, the classic mechanism of a lateral ankle sprain. This lateral perspective clarifies why anterior drawer testing targets ATFL integrity and why persistent talar tilt or pain anterolaterally can reflect incomplete healing, synovitis, or anterolateral impingement after sprain. For operative planning, the orientation shown matches the footprint used in Brostrom-Gould repair and helps distinguish ATFL fibers from the more posterior, vertical course of the calcaneofibular ligament.
