Description
Seen in lateral profile, the distal fibula (lateral malleolus) sits posterior and slightly inferior to the distal tibia, forming the ankle mortise around the trochlea of the talus. Spanning from the anterior margin of the lateral malleolus to the lateral aspect of the talar neck, the anterior talofibular ligament runs anteromedially in a near-horizontal band across the anterolateral ankle. Inferior to it, the calcaneus forms the subtalar partner to the talus, and the midfoot metatarsals extend distally in line with a neutral plantargrade foot. A small ligament with an outsized clinical footprint. Because the anterior talofibular ligament is the first lateral ligament to tension in plantarflexion and inversion, this side view maps directly onto the common mechanism of an ankle sprain during landing or cutting. Clinicians will recognize why tenderness localizes just anterior to the lateral malleolus, and why anterior drawer testing targets anterior talar translation when this ligament is compromised.
