Description
Seen from the lateral aspect of the right ankle, the calcaneofibular ligament (CFL) extends as a rounded, cord-like band from the anterior margin of the lateral malleolus of the fibula to the lateral surface of the calcaneus, coursing inferiorly and slightly posteriorly. Superior to it sits the distal fibula forming the lateral wall of the talocrural mortise, while the talus occupies a more medial and superior position between tibia and fibula. The calcaneus lies inferior and posterior, with the lateral subtalar region implied deep to the CFL; metatarsals and phalanges continue distally to provide orientation. Clear bony landmarks anchor the attachment sites. This lateral view matters because the CFL is the primary lateral stabilizer that resists inversion when the ankle is in neutral or dorsiflexion, and it is commonly injured with inversion sprains that also involve the anterior talofibular ligament. Clinical exam maneuvers that stress the subtalar and talocrural joints, and surgical reconstruction of chronic lateral ankle instability, both depend on appreciating the CFL’s oblique course to the calcaneus and its close relationship to the peroneal tendon sheath at the posterolateral ankle. Small structure, big consequences.
