Description
Running obliquely across the anterior distal tibiofibular interval, the anterior tibiofibular ligament (anterior inferior tibiofibular ligament) spans from the anterolateral margin of the distal tibia to the anterior border of the lateral malleolus of the fibula. The distal tibia and fibula frame the ankle mortise superior to the talar trochlea, with the ligament lying anterior to the syndesmosis and blending with the fibrous capsule of the ankle joint. Lateral elements of the hindfoot, including the talus and calcaneus, sit inferior and posterior in the field, with adjacent tarsals and metatarsals extending distally into the forefoot. Blue highlighting isolates key stabilizers in the anterolateral ankle. This anterior syndesmotic band is a primary restraint to external rotation of the talus and lateral shift of the fibula, so its integrity directly affects mortise congruency under load. A high ankle sprain often injures the anterior tibiofibular ligament first, and widening of the tibiofibular clear space on stress radiographs, CT, or MRI correlates with syndesmotic instability and occult diastasis. It also defines the surgical corridor for syndesmotic screw or suture-button fixation, where malreduction of fibular rotation can be as disabling as the ligament tear itself. Small structure, big consequences.
