Description
Oriented to the anterior aspect of the ankle, the anterior talofibular ligament (ATFL) is seen as a narrow, obliquely directed band spanning from the anterior margin of the lateral malleolus (distal fibula) to the lateral surface of the talar neck. Medial to it lies the talar trochlea beneath the tibial plafond, while inferior and slightly posterior the calcaneus forms the subtalar foundation. The distal tibia and fibula frame the ankle mortise superiorly, and the proximal tarsals and metatarsals extend distally into the forefoot. A small structure, big consequences. Clinically, this anterior projection is the clearest way to appreciate why the ATFL is the most commonly injured component of the lateral ligament complex in inversion and plantarflexion sprains, where the talus tends to translate anteriorly relative to the fibula. Tenderness localizes just anterior and inferior to the lateral malleolus, and the ligament’s course explains the positive anterior drawer test of the ankle when the ATFL is disrupted. Surgeons and sports clinicians also use this anatomy when planning arthroscopic evaluation for chronic lateral ankle instability and when placing anchors for a Broström-type repair, where the relationship of the ATFL to the talar neck and fibular tip guides fixation points. Use this asset in lower limb anatomy teaching (ankle mortise, talocrural articulation, lateral ligament complex), in sports medicine and orthopaedics materials on ankle sprain grading and instability, and in patient-facing education that needs an anatomically correct target for palpation and rehabilitation cues.
