Arising from the medial epicondyle and running distally in the superficial anterior forearm, the palmaris longus is shown transitioning into its long tendon as it approaches the wrist in a palmar (anterior) perspective. The tendon passes superficial to the flexor retinaculum and fans into the palmar aponeurosis, which spreads distally over the central palm toward the digits. Nearby superficial flexor elements are implied, with finger flexor tendons coursing distally toward the phalanges, while the metacarpals and phalanges sit deep to the soft tissues. Red muscle belly contrasts with the cooler-toned tendon and fascial sheet. Palmaris longus matters because its presence and morphology vary widely, and that variability influences examination, injection planning, and operative harvesting. Clinicians often palpate it by opposing the thumb and little finger while flexing the wrist, then track its tendon as a midline landmark just superficial to the carpal tunnel, where it can help orient a safe approach that avoids the palmar cutaneous branch of the median nerve. It is also a common donor tendon for grafting in hand surgery (for example, opponensplasty or ligament reconstruction), so understanding its relationship to the palmar fascia and flexor retinaculum prevents wrong-plane dissection.
