Description
Arising from the medial palmar aponeurosis and the flexor retinaculum, the palmaris brevis lies superficial on the ulnar side of the palm, immediately anterior to the hypothenar eminence and just lateral to the pisiform and hook of hamate region. Its short muscle fibers course transversely toward the dermis along the ulnar border, overlying the superficial fascia and running superficial to the hypothenar muscles (abductor digiti minimi, flexor digiti minimi brevis, opponens digiti minimi). An anterior palmar perspective also logically includes the palmar aponeurosis centrally, flexor tendons to the digits deep to it, and the ulnar neurovascular bundle passing in the superficial plane near Guyon canal. Small but specific. Clinically, palmaris brevis is one of the few muscles that inserts into skin, creating the characteristic dimpling and wrinkling of the ulnar palm during grip and protecting the ulnar neurovascular bundle by tensing the superficial fascia. This view helps clarify its relationship to Guyon canal, where ulnar nerve compression can produce sensory symptoms in the little finger and ulnar half of the ring finger with weakness of intrinsic hand muscles, while palmaris brevis itself may show painful, involuntary contractions in palmaris brevis syndrome. Its superficial location also makes it a useful landmark during ulnar-sided palmar incisions and dissection. Hand anatomy teaching benefits from a focused plate like this when introducing intrinsic muscle layers, palmar fascia organization, and the surface anatomy of the hypothenar region in gross anatomy, occupational therapy, and hand surgery coursework.
