Description
Running superficially along the ulnar side of the anterior palm, the palmaris brevis forms a thin quadrilateral sheet over the hypothenar eminence, lying immediately superficial to the hypothenar muscles and contiguous medially with the palmar aponeurosis. Its fibers arise from the medial border of the palmar aponeurosis and flexor retinaculum, then sweep distally and ulnarly to insert into the dermis and subcutaneous fascia of the hypothenar skin. Nearby landmarks in an anterior aspect typically include the pisiform and hook of hamate deep to the hypothenar soft tissues, the flexor retinaculum spanning the carpal tunnel, and the long flexor tendons entering the palm under the retinaculum. A small muscle, but easy to lose. Clinically, this view matters because palmaris brevis sits directly over Guyon’s canal, where the ulnar nerve and artery pass superficial to the flexor retinaculum between the pisiform and hamate hook, so edema, hematoma, or space-occupying lesions in this region can affect ulnar nerve function. Contraction wrinkles the skin on the ulnar palm and deepens the hollow of the palm, a protective mechanism that can be discussed alongside hypothenar soft tissue injuries and surgical exposure for ulnar nerve decompression at the wrist. The relationship to the palmar aponeurosis also helps clarify how superficial fascia and aponeurotic fibers blend in the heel of the hand.
