Description
Running across the anterior upper thorax, the superficial and deep venous channels converge toward the mediastinum to form the superior vena cava, with the right and left brachiocephalic veins joining just inferior to the manubrium. Each brachiocephalic vein arises posterior to the sternoclavicular joint from the union of the internal jugular vein and subclavian vein, the latter continuing laterally beneath the clavicle toward the axillary vein at the shoulder. Bony landmarks anchor the relationships: clavicle superior to the subclavian vein, first rib deep and inferior, and the rib cage, scapula, and proximal humerus framing the thoracic inlet. Short and direct. Clinically, this anterior perspective mirrors the corridor used for central venous access and for understanding catheter trajectories from subclavian or internal jugular puncture sites into the superior vena cava at the cavoatrial junction. The crossing course of the left brachiocephalic vein anterior to the aortic arch branches and deep to the manubrium explains why left-sided lines take a longer, more horizontal route and can be prone to malposition, while also clarifying how mediastinal masses or thrombosis can produce upper extremity and neck venous congestion.
