Description
Centered on the distal femur in anterior orientation, the articular surface is dominated by the patellar (trochlear) surface, a smooth groove bounded by the medial and lateral femoral condyles. The lateral trochlear facet rises more anteriorly and laterally than the medial facet, creating a bony buttress that guides patellar tracking during knee extension. Inferior to the trochlea, the condyles broaden to meet the tibial plateau, with hyaline cartilage covering the load bearing surfaces. Clean landmarks. Because the patellofemoral joint is where anterior knee pain often lives, an anterior view of the trochlear groove matters when teaching or planning for maltracking and instability. Trochlear dysplasia, a shallow groove and reduced lateral facet prominence, predisposes to recurrent lateral patellar dislocation, while chondral wear on the lateral facet commonly accompanies patellofemoral osteoarthritis.
